Patient came in with abdominal pain, Nausea and vomiting. This is not for a facility
but for a Physician based practice.

I was going to code this out as follows.. Please not I've only included the Hydration/IVP notes below..

99214.25,99354,96360,96361,j7030(ns),j2060 (ativan), 96375,j1885(toradol),96375,j2405 96375. Am I correct??




07/01/2011. ATIVAN 1mg diluted with 5 mL slow IVP over 1 minute. IV patency established. IV site checked: no pain, redness, or swelling. IV flushed thoroughly pre- and post-medication administration.


10:40 07/01/2011. IV FLUIDS STARTED: bag #1-1000 mL NS; at site #1 right antecubital space via IV pump (500 mL/hr started at 10:40)

10:35 07/01/2011. IV access: site #1, right antecubital space, 20g angiocath, with aseptic technique and good blood return; one attempt. Blood drawn: rainbow set. Labeled in the presence of the patient and sent to the lab.

10:45 07/01/2011. TORADOL 30 mg 10:45 slow IVP over 2 minutes via IV site #1. IV patency established. IV site checked: no pain, redness, or swelling. IV flushed thoroughly pre- and post-medication administration.

10:45. ZOFRAN 8 mg diluted with NS slow IVP over 2 minutes via IV site #1. IV patency established. IV site checked: no pain, redness, or swelling. IV flushed thoroughly pre- and post-medication administration.

11:15. COMPAZINE 10 mg diluted with NS slow IVP over 2 minutes via IV site #1. IV patency established. IV site checked: no pain, redness, or swelling. IV flushed thoroughly pre- and post-medication administration. Sedative drug warning given to the family.

13:00. IV FLUIDS STOPPED bag #1. Total amount infused = 1000 mL (1000 mL). --15:39
 
push/hydration

Hi. you should never use Hydration as your "initial" (96360). Code one of the pushes first 96374, additional pushes 96375, then the hydration 96361. If there was an infusion (15min - 1 hour) you would use 96365 as your initial.
