Hypertension and Conditions in I51.4-I51.9

Do hypertension and conditions in I51.4-I51.9 have a causal relationship or should they be linked by the provider? I am confused since the ICD-10-CM alpha index and tabular list "hypertension with heart involvement (conditions in I51.4-I51.9 due to hypertension) to report I11.9.
 
There is a assumed link between hypertension and heart disease/failure for code I50. and I51.4-I51.9 as long as documentation does not state the heart disease/failure is not associated with the hypertension.
