My patient has a diagnosis of Hypertensive Complication. The patient does have Losartan in the med list. However, there is no documentation supporting Hypertension as an officially diagnosed condition. Would you code this as I10? I want to because it seems like you would need to have hypertension in order to have a hypertensive complication... I also know never to assume in the world of medical coding. Help!
