Chapter 9:
3) Hypertensive Heart and CKD
Assign codes from combination category I13... when there is hypertension with both heart and kidney involvement.


We have a patient with hypertension and CKD Stage 3.

We are not kidney specialists and we don't want to step on the kidney specialist's toes.

If we don't want to use the code I13.. how can we document that his kidney disease is not caused by hypertension? Realistically, nobody knows.

Per the ICD - 10 -CM coding guidelines for I13 "Assume a relationship between the hypertension and the chronic kidney disease whether or not the condition is so designated."

I would code this
i13.10 (you don't mention heart failure)
N18.3

I don't see how this would be stepping the kidney specialist toes, as you are not managing the CKD only stating you are treating a patient with hypertensive heart AND CKD. The kidney specialist should use the same dx as you.
