Hypertensive part of Superbill

Just to confirm some dx(s) for our office superbill. Hypertensive cardiovascular disease with congestive heart failure is coded as 402.90 and 428.0. And congestive heart failure with hypertension is 402.91 and 428.0. Any feedback?

402.91 says "with mention of heart failure therefore both would be coded as 402.91 with the 428.0.

Thank you for the confirmation. How about Cardiomegaly with Hypertension? I feel it is a heart code plus 401.9. Any thoughts?

from the OC 1 2010 guidelines:
8. “With”
The word “with” should be interpreted to mean “associated with” or “due to” when it appears in a code title, the Alphabetic Index, or an instructional note in the Tabular List.
Therefor cardiomegaly with hypertension is 402.90
