In outpatient office setting is it appropriate to code E11.65 and E11.649 at the sametime?
The provider states both in the final assessment - uncontrolled type 2 dm with hyperglycemia and type 2 dm hypoglycemia.
also if they document patient showing nocturnal hypoglycemia and hyperglycemia during the day - what is the appropriate ICD-10 codes?
