Wiki I need a template/guide for Comprehensive exam

Does anyone have a template or guideline they are willing to share for meeting the Comprehensive exam for hospital Consult or Initial visits? My doc is providing the service, especially with the complicated trauma runs, but he is not dictating the required info to meet the Comprehensive exam. Thanks so much.
