Please help me code this with both CPT and DX codes in order please.:(:(


A 36 year old female complains of nausea and abdominal pain. She is diagnosed with Generalized Abdominal Pain probably due to Pancreatitis versus Cholecystitis.
 
I can't help with the E/M code because of lack of notes. Hint for the dx: look at your guidelines. If this is professional/physician side, you do not code "probable" or "possible" or "likely" conditions. In this scenario code the symptom.
 
Teresa is correct, the answers to this and the questions you have posed are in the coding guidelines. If this is part of a homework assignment and you need validation for your codes, you need to post your code choices as well. If this is a real note then you will need to post the entire note along with your code selections.
