Hi - I looked to see if this has been asked already and couldn't find anything. I am confused with the updated directions for 2026 in Chapter 9 Diseases of the Circulatory System. The chapter specific directions for Hypertension with heart disease was updated. The directions state:

1) Hypertension with Heart Disease
Hypertension with heart conditions classified to I50.-, Heart failure,I51.4, Myocarditis, unspecified, I51.89, Other ill-defined heart diseases, and I51.9, Heart disease, unspecified, is assigned to a code from category I11, Hypertensive heart disease. Use additional code(s) from category I50, Heart failure, or I51, Complications and ill-defined descriptions of heart disease, to identify the heart condition.

Hypertension with heart conditions classified to I51.5, Myocardial degeneration, or I51.7, Cardiomegaly, is assigned to a code from category I11, Hypertensive heart disease. No additional code is assigned to identify the specific heart condition.

The same heart conditions (I50.-, I51.4-I51.7, I51.89, I51.9) with hypertension are coded separately if the provider has documented they are unrelated to the hypertension. The applicable hypertension code I10, Essential (primary) hypertension, or a code from category I15, Secondary hypertension, should be assigned. Sequence according to the circumstances of the admission/encounter.


When you get to the code specific section it states that I11 Hypertensive heart disease INCLUDES I50.x or 151.4-I51.7, I51.89, I51.9

I11.0 code specific direction states to "use additional code to identify type of heart failure (I50.x)
I11.9 doesn't have "code also" directions

My question then is, why does the updated chapter guidelines call out "Use additional code(s) from category I50, Heart failure, or I51, Complications and ill-defined descriptions of heart disease, to identify the heart condition." when I51.x isn't a code range that is called out for either I11.0 or I11.9 per the code specific directions.

Am I missing something?

I see it as you must have a code from I50 to support I11.0, I13.0, I13.2. That is why the "Use additional code" direction is there. You may or may not have one of the other I51 heart conditions with hypertensive heart disease with heart failure.

It does seem the tabular would indicate to use an additional code to specify the type of heart disease "if applicable" as a reminder that the chapter specific directions state to use an additional code for I51 complications (except I51.5 and I51.7) to identify the heart condition. There are excludes 1 notes at I51.5 and I51.7 to remind us not to code them with I11 or I13.

The 2026 chapter specific directions are very confusing as they relate to hypertension with heart disease. Why are I51.4, I51.89, and I51.9 mentioned at the beginning of 9a.1. but then all I51 conditions need an additional code in the same paragraph (with I51.5 and I51.7 excluded in the second paragraph)? :unsure: And why use an additional code for I51.9? Isn't "heart disease" in the diagnostic description "hypertension with heart disease?" Using an additional code for I51.9 does not further identify a heart condition.
