I just completed the ICD-10 training for cardiology and I am confused as to why a few diagnosis listed were not coded and one that was coded as a current condition.

Question #4: Why was coronary artery disease not coded?
Question #8: Why was coronary artery disease not coded? They did code for the bypass graft.
Question #18: Why would you code for ventricular tachycardia as if its an active diagnosis when the records states "history of". Would you not assign a Z code?
