Good evening friends,

Scenario 1 : Inpatient EM Dx coding. How many Dx pts should a coder give for Inpatient Subsequent visit (Ex: CPTs 99231, 99232, or 99233) established problem: Diabetes w/hyperglycemia-IMPROVED (ICD 10 CM: E11.65)?
** Is this 1 improved established condition ---diabetes w/ hyperglycemia manifestation improved (1 pt)?
** 2 stable/improved established conditions --- diabetes, hyperglycemia (total 2pts)?
** A worsening established condition ---diabetes considered worsening because hyperglycemia manifestation is still present -regardless of improved status (2 pts)?
** 1 stable established condition (diabetes), 1 worsening established condition (hyperglycemia) (3 pts total)?

Scenario 2 : Inpatient EM Dx (established problems). How many Dx points should a coder give for IMPROVED Diabetes w/Foot ulcer E11.621 and then the addtional L97.4__ or L97.5__ for the type of foot ulcer?

Scenario 3: Inpatient EM Dx (established problems), How many Dx points should a code give for IMPROVED Hypertension w/ CKD (I12._), Diabetes w/CKD E11.22, and then the addtional N18._ for the type of CKD?

Here are the resources I have reviewed so far but it is difficult to find anything that gives clear guidance.
