Is there a guideline that helps to order ICD-10 in order of severity on the claim. Does that have to come from the physician? I have an ins carrier telling us that our ICD 10 are not in order of Severity. Please help.
 
What kind of diagnosis are you coding for? I believe for fractures you have to code highest severity first. Otherwise, I believe all other ones are based on the reason of encounter l, with the exception of those certain conditions that have guidelines on themX
