What ICD10 would you assign to the following: Any help appreciated.
1. FIBRILLARY GLOMERULONEPHRITIS WITH ACTIVE CRESCENTS AND
SEGMENTAL SCLEROSIS (SEE COMMENT)
2. ACUTE TUBULAR INJURY AND INTERSTITIAL NEPHRITIS, LIKELY RELATED TO
#1
3. ARTERIAL AND ARTERIOLAR NEPHROSCLEROSIS
COMMENT: The fibrillary glomerulonephritis is associated with 18% active (cellular)
crescents. The chronic glomerular changes include 48% global glomerulosclerosis and
10% segmental glomerulosclerosis. Considering the reported clinical history, it should
be noted that fibrillary glomerulonephritis can be associated with HCV infection
[Reference: J Am Soc Nephrol. 9 (12):2244-52, 1998]. There is moderate tubulointerstitial
scarring. Clinical correlation is recommended.
 
Could N05.7 or N05.9 work for #1? I do not code nephrology, but these seem reasonably logical....anyone else? (looked up Glomerulonephritis / crescentic ; and sclerosing)
