I have a question on diagnosis order. If the patient is in for a well and has a sick visit due to finding by the provider. I am aware that you would use the well with abnormal finding dx along with the finding dx. When you charge the well level and the sick we usually put the well code on the well and the other code on the sick visit. Well with ICD-10 it says to put the abnormal findings code with the well code. Would you put the abnormal finding code example otitis media on the well visit as well as the sick visit or would you continue to place the codes like we did prior to icd-9?
