ICD-9 code help - with a diagnosis code

I am still looking for help with a diagnosis code that is considered medically necessary when using a 36561 ir 36569 CPT code for PICC line insertion. The LCD list I printed is very limited, and my surgeon has instructed ME to figure out which code to use.
 
Pre-op and post-op diagnosis both state the type of cancer + "requiring chemotherapy and vascular access".
