The MD didn't give me any specific infection, sorry about that. I truly appreciate you trying to help me.

You might try to go back to the physician and explain to him/her that it is essential that you have proper documentation in order to code appropriately. I know it may be uncomfortable at first but in the end it will make you a better coder and it may also develop a relationship with your physicians that will make it easier to communicate in the future. Also, it is YOUR butt on the line when you code. You need proper documentation in order to code a chart properly and any physician should understand this. Just a thought...
