Wiki icd-9 issue

when giving a dx code for
bx of small bowel ( - malignant carcinoma
bx of small bowel ( 10 cm from ileocecal valce)- malgignant Carcinoma
etc.....
.these are from different parts of the small intestine with 5 different bipsy sites. all of which is carcinoma. so when coding for diagnosis
should we give one ICD 9 for all the 5 biopsies like 197.4. or
do we have to give 5 X 197.4...?
pls help

This actually in the guidelines, you only use a dx code once on a claim never use the same code more than once.
