How would you code, "headache, appears to be neuralgic". This is all I have and then Plan is to continue neuro checks and do MRI

Codes to 784.0, headache; unspecified. On a sidenote, "appears to be" denotes some uncertainty so, you would not code something written as "appears to be neuralgic headache". Since it is written as "headache, appears to be neuralgic", you are able to code the headache as unspecified. Hope this makes sense/helps.
