When coding a physician's sevices provided during a inpatient hospitalizaiton, which set of ICD-9CM coding guidelines are applicable, the inpatient or outpatient?

I'm particulary interested because of the guidelines related to inconclusive diagnoses (probable,suspected, rule out).

Outpatient ICD-9 Guidelines always apply to physician services. It's easy to miss that statement in the guidelines, but it's there.
