For the ICD Medicare NCD policy 20.4....

When performing a generator change due to battery end of life, do you base your indications/requirements on the patient's current status or the patients status at the time of the initial implant?

I use V58.31 - PM or V58.32 - ICD along with the indication(s) for device; for my physicians this is usually SSS 427.81.
