What CPT codes would you use for a upgrade from a dual pacer to a dual ICD and a new bi-ventricular?

That all depends on what they actually did. You would have a removal code for pacemaker and then an implant code for icd plus a LV lead code?

There is a good chart in your CPT, my book it is page 187( CPT professional edition) that you might be able to use. If you post more info here I will be able to give you exact codes.
