What about a ICD10 code for a non-smoker? There's no history as a smoker. I was not able to find one.

No, there is not a code in ICD-10 for patients who have NEVER smoked. When that happens do not put anything on the claim (there's no code to report)
Respiratory codes (Jcodes) are one set of codes that require smoking codes to be reported if the patient is a smoker, exposed to smoke or history of smoking. If the patient has asthma (for example) and they smoke cigarettes you would report the appropriate J code for asthma and F17.210 Nicotine dependence, cigarettes, uncomplicated.
If the patient has asthma, but doesn't have any smoking history or exposure, simply report the appropriate J code for the asthma.

For I25-CAD codes, is it more important to post the smoking code secondary to the CAD or the more significant cardiac codes treated on the visit and the smoking codes last?

Z code would go at the end. I think of it the same as V codes in 9 going at the end unless it was the reason for the encounter in absence of an illness. Its really just information only for tracking purposes for those who deal with the statistical analysis.

What denial reason is it giving. This looks appropriate. Maybe its just not a covered benefit?

Maybe you need a counseling code first listed such as Z71.6.
