How would you code long time previous to back use? Z72.0 for use or code as history of?
 
Z Codes

Z codes represent reasons for encounters. They can be used when a person may or may not be sick or when some circumstance or problem presents which influences the person's health status but is not in itself a current illness or injury.

If the doctor wrote "previous" it seems to indicate "not current".

Z72.0 is for "current tobacco use" not otherwise specified.
F17.2 is for nicotine dependence
Z87.89 is for "history of" nicotine dependence and excludes current dependence.

So the question to determine the code is whether this person has stopped or is currently using.
