Does anyone out there have experience with credentialing a IDTF with CMS?

Questions: Do you only need one supervising physician? I know he does not need to be on site but what are the guidelines if he is on vacation or out for an extended time? Could probably use a phone call on this. If you would leave your contact info I would greatly appreciate it. Thanks
