My foot specialist utilizes the Iliac Crest Bone Marrow Aspiration and Graft in his Subtalar Fusion procedures on a regular basis.

I know the coding changed a few years back and now to report that procedure, it is suggested to use the 20999 unlisted code.

We have been doing that and have continually been denied and denied on appeal when detailing the description of the procedure.

What are we doing wrong?

Or is this a procedure that is billable and not payable under the circumstances?

Is there any reference material on the subject someone can point me in the direction of?

Lol,I figured as much I get that as well.Between the 2020 CPT Book and the Supercoder definition with expansion.(the quoted "Tips" below)

"Tips
The provider reporting this code obtains bone marrow for diagnostic purposes only. This procedure does not involve obtaining a sufficient amount of bone marrow for transplant purposes. Do not separately report this code if a provider performs this service at the same time as harvesting bone marrow for transplantation."

That brings me back to the 20999 unlisted code from the CPT book and we're not getting paid using that.
 
I'll have to pull out my specialty book out as well and look for that. My Coder III taught me to use 38220 and it just sounded odd at the time. I hope someone out there in the coding universe can help because I code for foot and ankle specialist too. I'll see what I can do as well.
 
CPT code 28725 (Arthrodesis, subtalar) does not include bone marrow aspiration 38220 so you could report the two codes together. I thought the same about the use of 38220 with an Arthrodesis, this question has been brought up and has been verified that the two codes can be billed together. I have had no problems billing this out this way. I hope this gives some help.

None of the orthopedic surgical procedures "include" the injection of BMA, but it's frequently done to promote healing. Thanks for posting that information from Codify, we have always billed 38220 for obtaining the BMA.
