Does anyone know where I can find guidelines or resources about billing/not billing for implants that are implanted, then explanted in the same session?

I have been told that Medicare is very vague on implants and many payer contracts don't specify either way.

A third-party implant billing company is saying, " if it touches the patient, it is billable".

I can't find anything in writing.
