Patient comes in for an office visit with nausea. Doctor has the nurse give an injection of Zofran for the nausea.
Do I attach this charge to the doctors E&M? I can't separately bill out in the nurse's name, our EHR is not allowing that.

The documentation is the deciding factor but this can be billed one of two ways:

1. The administration of the injection can be considered as inclusive to the E&M

or

2. The administration of the injection is significant and separate from the E&M and can be billed with a 96372 and the E&M with a modifier 25.

Either way both codes are billed under the physician as the nurse is working according to their orders.
