When would you consider a tobacco smoker as in remission vs history of smoking tobacco?

Keep in mind to always go by what the provider documents and how the documentation may explain the history of smoking. Example, Pt used to smoke 2 packs a day until 5 years ago and hasn't used tobacco products since that time.

The patient is in remission if the provider says they are. It's a history of smoking if the provider says it's a history of. If in doubt, query the provider.
