Can a new patient be billed as "incident to" if the physician only reviews and agrees with the treatment plan but is not involved in the History, Exam portion of the visit? How involved does the physician have to be to bill a new patient "incident to" ?
 
No a new patient can be billed by the physician using the physician's NPI or by the NPP using the NPP's NPI. A new patient can never be billed using incident -to guidelines.
