We are in the process of auditing the ?incident to? notes dictated by the physicians in the practice. I wanted to confirm the guidelines for new patients and established patients with new problems. My understanding is that a PA or other healthcare professional can enter the review of systems and the medical history, but the physician personally must discuss the history of present illness with the patient, perform the exam, and develop the plan of care in order to bill under his/her NPI. If the PA does the HPI or the exam or the plan of care, then the PA?s NPI must be used. Is this correct?
