If the PA orders a CT scan for patient with breast CA does that change the plan of care and nullify the incident to factor? Would imaging be part of the routine plan of care as blood work would be?
 
One opinion

I would look at the original treatment plan to see if these tests were included in that plan. If not, I wouldn't bill the visit as incident-to, as it appears that the PA/NP is addressing more than the condition being treated, even if this is an annual assessment/screening.
