Wiki Incorrect MRI filed in patient record now on microfilm. what to do?

First off we do not have an HIS manager. We have an MRI study on a patient done in 2001 that is incorrect. Evidently an elderly patient was stuck in the mobile MRI scanner and that patient's results got attached to our patient's file.

Our patient had the MRI done again and it is correct, her initial MRI results that are now on microfilm were the wrong results. At the time the assistant CEO was approached about this but did nothing. Next the HIS manager in the department was approached later and she also did nothing.

At this point it is evident the microfilm record needs to be ammended. How do we go about doing this in a proper manner.

If any of the RHITs out there have dealt with a situation like this perhaps they or anyone can give some guidance.
