Hi Folks,

I am reviewing documentation requirements for my psych department. We have written that a treatment plan is required to be updated every 90 days, does this hold true for everyone else as well? How does everyone else handle the treatment plan in their practices?

Yes, we require the treatment plan to be updated every 90 days, it's a Maine care requirement We have a template of required information, target symptoms, goals, frequency/duration of treatment, capacity to participate and progress. In our EHR, the treatment plan is part of the actual note as there was no way to do them separately.
