can an infusion (96365) be billed if documented at 30 minutes or does it have to be documented at 31 minutes or more to bill the first hour (I thought I saw something in writing somewhere stating that it had to be at more than 50% of the first hour to bill this code, but I cant seem to find it now). Thx :-)

so now I'm confused....There is a CPT assistant (Oct. 2011, Reporting Guidelines for Time Based Codes), which state "A unit of time is attained when the midpoint has been passed. For example, an hour is attained when 31 minutes have elapsed (more than midway between zero and sixty minutes)

Is this not correct?
