When billing Medicare for Chemotherapy infusions and a drug is mixed with 1000 cc of D-5-W, is the infusion of HCPCS J7070 billable?


Example:
leucovorin 588 mg in D5W 250 mL infusion


Is it appropriate to bill J0640 (Injection, leucovorin calcium, per 50 mg) with J7070 (Infusion, D-5-W, 1000 cc)?
 
Nope, CPT guidelines state that the fluid used to administer the drug is incidental and not separately reportable. Now if it was pre or post hydration, that may be reportable depending on the amount of time attribtuted to the hydration.
