When billing for IV-Infusions, if using a combination of drugs/substances as part of the infusion (for example, the Meyers Cocktail) do we only need to report the timed based service codes since all drugs are administered from start to finish or since there is more than one drug being administered do we need to also code the sequential drug infusion?

This question is in reference to the following codes.

96360 IV-Infusion, hydration, Initial, 31 min. - 1 hour (Hydration)
96361 IV-Infusion, each additional hour (Hydration)
96365 IV-Infusion, for therapy, prophylaxis, or diagnosis, up to 1 hour (drug)
96366 IV-Infusion, for therapy, prophylaxis, or diagnosis, ea. additional hour (drug)
96367 Additional seq. infusion of new drug/substance, up to 1 hour (drug)
96368 Concurrent Infusion (1 site, 2 lines, infusing @ once), Once per day (drug)
96369 Subcu Infusion, Therapy/prophylaxis, initial, up to 1 hour (drug)
96370 Subcu Infusion, Therapy/prophylaxis, Each additional hour (drug)
96371 Additonal pump setup w/ establishment of new Subcu infusion site (drug)
96372 Therapuetic, prophylactic, or diagnostic injection; Subcu or intramuscular (drug)
96373 Therapuetic, prophylactic, or diagnostic injection; Intra-arterial (drug)
96374 Therapuetic, prophylactic, or diagnostic injection; IV push, single or Initial substance/drug (drug)
96375 IV push, Each additional Sequential IV push of a new drug/substance (drug)

Since the Myers Cocktail is a mixture of substances and not each substance being administered individually, then this would accounts for one administration and not sequential administrations of nutrients.

Number of billable units for 96367

We are a chemotherapy / infusion office. We have been receiving refund request for billing 96367 4 times especially with FOLFOX regimens. This has not been an issue until recently. Did something change?

Pre Meds are given in seperate bags ( not mixed) :
Zofran
Benadryl
Decadron
Emend

Is anyone else having this issue?
