Included in what? If you are talking about fracture care cases then yes the initial application of cast is included in fracture care service. You would bill an E/M-57 + Q codes for supplies + Fracture care code.

Jeanne, your supplies are always seperately billable, whether you bill:

1) ala-carte E&M - 25, Cast code -rt or -lt, and your supply code, Q-code or A-code, depending on the ins.

2) E&M - 57, Closed Treatment Code with -rt or -lt, and your supply code. Mary
