Wiki Initial hospital consultation and ECHO on the same day

Several of our private insurance companies have suddenly decided to not pay for an ECHO when it is done the same day as an initial hospital consultation or a subsequent hospital visit. The consultation/visit is done at a different time but the same day. We append modifier 25 but the claims deny as unbundled not allowed; we send appeals but they are also being denied, is anyone else having this problem? any suggestions on how to get these paid would be helpful.
