Hi,

I would like to ask for your input regarding coding initial or subsequent MI.

I have a patient who was admitted due to SOB. Patient had an NSTEMI 2 weeks ago. Later during the admission, patient became bradycardic and reported chest tightness. A code was called . Monitor showed bradycardia then went to PEA. Then the family decided to stop. On the physician's documentation, it said "Acute MI progressing to arrhythmia possible sudden PE, inferior MI progressive." The death documentation also stated "Acute Cardiac Arrest' and "Acute MI". Is this a case of an evolving NSTEMI to STEMI or a subsequent MI?

Thank you.
