OK I have been through and through my I-10 book and cannot seem to find where they are defining or directions as to WHAT initial vs subsequent means.....

can someone direct me to where you are finding this info? initial = active ?

If you have the AAPC ICD-10 manual, the information you're looking for begins with the last paragraph of the first column (it's under the Chapter 19 information) on page 64.
