What is the guideline for cpt 20610 when a doctor aspirates the knee and then injects the knee with medication? Do we code 20610 once or twice? Any help would be good. Thanks.

You can only charge that code once per site. It isn't unusual for a doctor to aspirate a small amount of fluid from a joint before he injects the same joint, but don't charge it twice!

I concur with bfontaine. Since it's the same site, you can only charge the code once.
