Can someone instruct on the correct way to bill for injected materials?

Synvisc J7325 - 1 mg (is it correct to always bill 16 units?)

Bethamethasone Acetate - J0702- 3 mg

Kenalog - J3301 10 mg

Kenalog - J3302 5 mg

Kenalog - J3300 - 1 mg

Orthovisc - J7324 - per dose as per HCPCS book.
