I am a Hospice coder. I have had two cases recently that involve a severe injury causing impending death. 7th character A, initial, does not seem to fit because the initial treatment has ended, but D does not seem to fit either because we are not treating a healing injury. The terminal dx for the Hospice admission is traumatic brain injury but what 7th character best fits the dx?
 
Sounds like S is the appropriate 7th character. However you need to code first the effects of the TBI that has necessitated the hospice care. You need the effect the TBI is the cause.
