Wiki injury to coronary sinus during retrograde cardioplegia

:eek:I need some assistance. I am new to cardio-thoracic surgery coding. My provider was cannulating for cordiopulmonary bypass to perform bypass grafting. During the cannulation, the coronary sinus was lacerated by the balloon on the cannula. He then repaired the laceration and continued the procedure. My question is, can/should he bill for a procedure, the need for which was caused (however unintentional it may be) by an injury caused by him. If so, what would be the appropriate CPT/ICD9 combination to bill?
