Good day all! would anyone be so kind to help me out with a situation.

We have a patient that was admitted 3 days prior and the dr discharged the patient and admitted the patient on the same date. I asked the dr why he did this and he said that is was because he needed to admit the patient in as a hospice. I know that we cannot bill for a discharge and admit on the same date.

Can someone please tell me how i can go about charging this visit?

patient admitted 06/10/08
patient became hospice 06/13/08

Can I bill for an initial care on 06/13/08 and append a GV modifier or should I bill a subsequent visit with a GV modifier?

and I know the discharge is out of the question since it is not possible to charge this.

I would say it should just be a subsequent visit with the GV modifier. If the provider did more work, took more time with patient (ie, paperwork to admit to hospice) then he might be able to increase the level of visit.
