Hello, I'm a surgical coder not that great with E/M. I have a provider that is asking the following...

"We often get consulted at night on a patient and we have to go through the chart and labs and write a note of initial assessment, make changes in their medications and fluids and talk to the nurse and the attending and it almost takes us an hour or more, yet we have not seen the patient or examined the patient. Can we charge for that service and what is the code for that?"

Can anyone help please? Are there resources you can point me to?
