I need some input/sources on diagnosis codes for inpatients for each day they are in the hospital. I.E. Do I put what the provider documents for that day as their diagnosis? or just the final diagnosis for the whole stay on each day? What if the patient had pneumonia but they didn't document it until day 3, what do I put for the first two days?

I also need a source of where to find this information for my biller, any help would be greatly appreciated!
