I have patient case here that reason for admission debridement of the non healing wound of lower extremities underlying condition of diabetes, also he had atherosclerotic heart disease with pacemaker, hypertension, hyperlipidemia, diabetic neuropathy, urinary retention etc.

E/M level evaluation:

1. Detailed History
- Extended HPI
- Expanded problem focused ROS (7 ROS)
- Complete History

2. Comprehensive exam
3. High Complexity MDM

99221, 99222 or 99223?
 
Initial inpatient E&M codes require 3 of 3 components to be met. If the history is detailed, then 99221 is the correct code, even though the other components may meet the requirements for the higher levels.
