I know I should know this, but I just can't seem to make a decision. I am recoding a consultation for a Medicare patient. I used the worksheet and came up with the following: Comprehensive - History, Detailed - Exam, and Moderate - Medical Decision Making. I know for the initial inpatient visit you need to meet or exceed 3 components. So with these levels would I choose a 99221 or a 99222?
 
99221, because of the detailed exam. For 99222 and 99223 you need comprehensive History AND a comprehensive Exam. For 99222 mdm, moderate and 99223, mdm high, however both do need comprehensive history and exam.

For initial hospital care codes 99221-99223, these require all 3 components.
