I work at an insurance company and we are bringing our inpatient chart reviews in house. This is a new process and we are unsure how to break these reviews down. Do you do the review from ER visit through to the discharge summary as one encounter or do you break down each DOS or each provider visit? We are trying to make this as streamlined and quickly as possible. Thank you
